We4Care: five portals around one care plan

We4Care supports family caregivers in Switzerland through chronic or complex care journeys, pairing a shared-care-plan tool with a dedicated human facilitator. Care coordination fails at the seams between people, so the product models the people, not just the plan.

What the founder described does not sound like a software problem at first. A patient and the professionals treating them hold different pictures of the same illness, and the family member keeping it all together has no standing in either of their systems. Swiss healthcare is well resourced and still loses things at the handover, because the handover is where nobody's record reaches. So what was needed was not an app for patients or an app for clinicians. It was one record with a separate door for each person entitled to walk through it.

Five roles, five portals

• Patient, caregiver, medical provider, administrator — and a Copilot portal for the facilitator who coordinates between them.

• Each role sees its own slice, which is what keeps a shared record from becoming a privacy problem.

The care record

• Ongoing care plans with support objectives and key milestones.

• A daily journal, symptom logging, notes and care history.

• Tasks and a shared calendar across the care team.

Medication, tracked properly

• Medications with reminder logs, missed doses and side effects recorded separately.

• SMS reminders, because the person who needs the reminder is often not the person holding the app.

• PDF export of the care plan, for appointments where paper is still what happens.
